چکیده

The effects of primary and secondary polycythemia on circulatory and ventilatory events have been the center of much investigation. There has, however, been sharp disagreement about the clinical merit of phlebotomy in cases of secondary polycythemia. Howarth, et al.' and Lewis, et al.,' among others, have concluded, although for somewhat different reasons, that therapeutic phlebotomy is detrimental in patients with polycythemia and lung disease. Cournand,3 on the other hand, has stated that polycythemia is responsible for some of the pulmonary hypertension and phlebotomy constitutes an important aspect of treatment. In attempting to explain the physiological effects of disease states such as polycythemia vera or secondary erythrocytosis, it would be of considerable value to know the response of normal subjects to an increase in hematocrit without associated changes of blood volume or pulmonary disease. With this objective in mind several cardiorespiratory parameters have been monitored in dogs rendered acutely polycythemic but whose blood volumes remained normal.

Effects of polycythemia and anemia on cardiac output and other circulatory factors’

RICHARDSON, TRAVIS Q. AND ARTHUR C. GUYTON. E&xts of polycythemia and anemia on cardiac output and othar circulatory’ factors. Am. J. Physiol. IgT(6): I 167-1170. I g5g.---Normovolemic anemia and polycythemia were studied in 14 dogs. Cardiac outputs increased with anemia and fell with rises in hematocrit. Although many factors-such as chemical changes-may play an important role in these variati...

Hemodynamic response to normovolemic polycythemia at rest and during exercise in dogs.

Very little is known about the influence of polycythemia on oxygen transport during exercise. We studied chronically instrumented dogs trained to run on a treadmill before and after their hematocrit had been increased by isovolemic exchange transfusion with packed red blood cells. With normovolemic polycythemia, cardiac output fell in a linear fashion as hematocrit was increased to 65%, but the...

Polycythemia and high levels of erythropoietin in blood and brain blunt the hypercapnic ventilatory response in adult mice.

Changes in arterial Po2, Pco2, and pH are the strongest stimuli sensed by peripheral and central chemoreceptors to adjust ventilation to the metabolic demand. Erythropoietin (Epo), the main regulator of red blood cell production, increases the hypoxic ventilatory response, an effect attributed to the presence of Epo receptors in both carotid bodies and key brainstem structures involved in integ...

Adverse effects of normovolemic polycythemia and hypoxia on hemodynamics in the dog.

Although polycythemia commonly occurs in hypoxic patients, most hemodynamic studies of polycythemia have been done in the normoxemic state. We studied the combined effects of polycythemia and hypoxia. In 11 splenectomized, anesthetized dogs, hematocrit was increased from 42.6 ± 1.2 to 65.5 ± 0.6% (SEM) by isovolumic exchange transfusions with fresh canine packed RBC's. Studies were conducted du...

Perinatal hypoxia increases susceptibility to high-altitude polycythemia and attendant pulmonary vascular dysfunction.

Perinatal exposures exert a profound influence on physiological function, including developmental processes vital for efficient pulmonary gas transfer throughout the lifespan. We extend the concept of developmental programming to chronic mountain sickness (CMS), a debilitating syndrome marked by polycythemia, ventilatory impairment, and pulmonary hypertension that affects ∼10% of male high-alti...
